5.2 Recording and Reporting Tools
The following recording and reporting tools are to be developed and used for the COVID-19 vaccine introduction.
- COVID-19 immunization register.
- COVID-19 immunization tally sheet.
- COVID-19 immunization card (home-based record).
- COVID-19 immunization reporting form.
- COVID-19 vaccine stock record and vial retrieval form.
- AEFI line listing, reporting/notification, and investigation form.
- RAT/checklists.
- Supervision checklists.
Immunization Register
- The Immunization Register is used to record individual identification information and vaccine doses given to an individual.
- It helps health workers keep track of each dose that has been given and the completion of the scheduled doses.
- They can also link vaccination data with other medical information, such as COVID-19 test results.
- The register will be to track defaulters.
- The data in the register can also be used as a source of information during program evaluation and surveys.
Tally Sheets
- Tally sheets are used to record all doses given during an immunization session.
- They are used to count the number of vaccinations administered during a day, week, month, immunization session, or campaign day.
- They are also useful in tracking doses administered and any vaccine doses wasted.
- They should allow for tallying of COVID-19 vaccinations disaggregated by:
- Place
- Age
- Sex
- Comorbidity/existing risk conditions.
Immunization Cards
- Enable health workers to determine which doses are due.
- Serve as a reminder for the recipients when the next visit/dose.
- Serve as documented proof of immunization status.
- Serve as proof of vaccination for individual’s travel, educational or occupational purposes.
- Provide vaccination information in case of AEFI or positive COVID-19 tests.
- Facilitate coverage surveys.
Reporting Forms
- This will help to share information to higher levels, monitoring the COVID-19 vaccination coverage, as well as the overall progress throughout the vaccination period at each level.
- The reporting forms will be prepared by level which include: vaccination team reporting form, Kebele reporting form, and Region/Zone/Woreda reporting form.
