The level of consciousness (LOC) should also be assessed as a vital sign.

AVPU scale is a rapid method of assessing LOC. (See the previous session for more detail).

A = Alert and awake.

V = Response to verbal stimulus.

P = Responsive to pain.

U = Unresponsive.
